Dearest PARENTS:

Welcome to the Academic Year 2018-2019!

As part of the Guidance and Counseling Services, we are currently filing our students’ Personal Data Inventory and administering the OLSAT 8.

We distributed the Personal Data Inventory Form and Parental Consent Form for OLSAT 8 assessment to your children last September 6 (Thursday).  Kindly accomplish the forms and return on September 10 (Monday), in sealed envelope (for confidentiality purposes).
